Servicing all over Victoria and New South Wales, our team, led by Agnes Bamford, have developed a vast network of support services in the community. We are highly proficient in coordinating a wide array of supports, whether it be community, mainstream, government, or other services that you may need to reach your goals. Our team will guide you through the process of connecting with, selecting, and receiving services from your preferred service providers. 

Additionally, we aid you in budget allocation for each type of support to ensure your NDIS funding is being used optimally and not being under or overspent. As your Support Coordinator, we will support you to express your preferences and make decisions. We will empower you to express your agency, connect you with the services you need and ensure a smooth journey through the NDIS.

Specialist Support Coordination

We also provide Specialist Support Coordination to those participants that have this allocated in their NDIS plan. Specialist Support Coordination is a higher level of support for participants that require support with more complex needs or situations. The focus is on reducing the complexity in the participant’s environment and helping them overcome any significant barriers to implement their NDIS plan and achieve their goals.

What is Support Coordination?

Support Coordination assists you in comprehending and utilizing your NDIS plan to achieve your goals. It facilitates connections with NDIS providers, community resources, mainstream services, and other government agencies, while also fostering your confidence and proficiency in managing and coordinating your supports.

What are the different levels of Support Coordination?

There are three levels of Support Coordination that can be included in your plan. If you have Support Coordination in your NDIS plan, the level allocated is decided by NDIA and stated in your NDIS plan. The three levels are:

Support Connection – This support is to build your ability to connect with informal, community and funded supports, enabling you to get the most out of your plan and achieve your goals.

Coordination of Supports – This support will assist you to build the skills you need to understand, implement and use your plan. A Support Coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, live more independently and access your community.

Specialist Support Coordination – This is a higher level of support for people in complex situations. A Specialist Support Coordinator will assist you in managing challenges in your support environment and overcoming any barriers to receiving the supports you need.

How much does Support Coordination cost?

Support Coordination is funded within Capacity Building – Support Coordination in your NDIS plan, there is no out-of-pocket cost. The number of hours allocated for a Support Coordinator will vary for each participant and is decided by NDIA.

Can my Support Coordinator attend meetings with or for me, including with NDIA?

Absolutely! For meetings with NDIA, please be sure to let the NDIS Planner know to include your Support Coordinator in all emails and discussions.

Can my Support Coordinator help with an NDIS plan review meeting?

Definitely. Our Support Coordinator can help navigate the whole process. Before the meeting, we will produce a Plan Review Report to outline all services arranged, and get the necessary reports from any allied health professionals that are involved. During the meeting, we will support you to ensure NDIA gets all the information they need to make an informed decision on the Plan Review. After the meeting, we can act as the point of contact for NDIA.

Are Support Coordinators and Plan Managers the same?

No. A Support Coordinator’s role is to implement and guide you through your NDIS journey, to help you achieve your goals. A Plan Manager only handles the payments for services you receive.
